Description |
From the evidences, it appears that a high velocity concentrated flame had struck the jet pipe from inside at a steep angle causing dislodgement of sheet material and formation of an opening on the inner superalloy heat shield. The melting of the outer stainless steel heat shield and the sheet metal around the starboard hydraulic jack of the power ring had occurred following the formation of this opening. Analysis showed that opening up of a nozzle at the base of the AB manifold could cause a high-pressure fuel jet resulting in a high velocity flame which could cause localized heating on the jet pipe. This needs to be corroborated with the location of the fatigue failed nozzle and the opening on the jet pipe. Probably, a simulated experiment would help in resolving this aspect. The damages seen on the HPTR and LPTR blades, and IInd stage NGVs are secondary in nature and subsequent to the primary failure.
